Meadows Short Term Rehab Services

From illness to injury or short- to long-term rehab, we’re here to help you get well and get back to your life.

Our 24-hour, resident-centered short-term rehab services focus on an individualized approach to one’s recovery from a planned procedure or recent hospitalization. With our full scope of clinical and therapy services, our aim is to help the resident recover and increase their function to the highest level possible.

Our clinical staff and occupational, speech, and physical and therapists provide treatment programs for a variety of physical and medical diagnoses.

Services Available & Amenities

Short-term rehab services are provided within the Meadows Health Care Center, with access to a fully equipped therapy gym.

Services include:

  • Pre-operative planning and program consultation
  • Physical, occupational and speech therapy as prescribed
  • Access to fully equipped therapy gym with therapeutic modalities including ultrasound, electrical stimulation and diathermy
  • Customized care plan and discharge planning based on individual/family goals
  • Personalized pain management program
  • Ongoing education and training provided for self-management of an individual’s needs
  • Family and caregiver training and education
  • Home evaluations to ensure a successful discharge
  • Opportunity to continue the relationship built with therapist on an outpatient basis
  • Supportive services, resident advocacy, family support and referrals
  • Expansive enrichment, recreational, educational, social, spiritual and wellness programming

Accommodations & Amenities include:

  • Fully furnished semi-private and private accommodations with en suite bathroom
  • Three chef-prepared nutritious meals, plus daily snacks
  • Daily housekeeping, towel, linen service and personal laundry
  • 24-hour emergency response system
  • In-room flat-screen television with cable service and wireless internet
  • Telephone service available (fee)
