Bayada Home Health Care brings more than 40 years of compassionate, in-home health care to Tel Hai. There may come a time when a resident needs additional health care services, and Bayada offers two services in the comfort of the resident’s home, including:

• • Skilled Home Health Care: For medical and physical needs, Bayada nurses, therapists and social workers collaborate to provide skilled services. They work together with Tel Hai’s care team to reduce falls, prevent unplanned hospitalizations and ultimately help residents to age in place with good health. Since Bayada is capable of treating residents under Medicare Part A, Medicare Part B, and other private insurances, residents will receive the right care at the right time, under the right insurance benefit.
  • Hospice Care: Bayada offers comprehensive hospice services including social, medical and spiritual care to support the resident and caregiver. Hospice care improves the quality of life and provides comfort and dignity while alleviating symptoms and stress.

Continuous Home Care, Inc. was founded in 1995 with the focus of providing high-quality, cost-effective, and personalized care to those in need of medical services in the home setting. CHC is goal-directed, care is based on the needs of patients and is evaluated in terms of goals which are mutually set with the patient and family. Continuous Home Care provides home health care, skilled nursing, physical therapy, occupational therapy, speech language pathologists, medical social workers and home health aides.
